Hardship Forms - Hardship forms will be honored for two weeks beyond the Drop/Add period. After that time, if exceptional circumstances exist, the Department Chair and faculty member will make a determination regarding the request. No hardship form will be signed after two weeks beyond the Drop/Add period without first discussing it with the Department Chairperson.
Withdrawal Forms - students should check the Registrar's webpage because submission dates will change each semester. The following dates are for Fall 2007 semester.
- Withdraw Form A (For W only) due on or before October 22, 2007 - no signature required.
- Withdraw Form B (For WP/WF) due on or before November 26, 2007 - professor's and department chairperson's signature required.
- After November 26, 2007, the Registrar's Office will no longer distribute this form; it will only be available from the Dean of the College (College of Education) in which the course is offered. During these last four weeks of the semester, withdraw must be considered exceptional and may occur only with the approval of the professor, department chairperson, and the college dean and only for good and sufficient reasons beyond the control of the student.
