To arrange this option, professor’s approval is required. “Professor’s Authorization to take class Pass/No Credit” forms are also available at the Office of the Registrar.

Withdraw

• Withdraw Form A (For W only) - (Fall 2008 - Due on or before Monday, October 20, 2008 with the Professor's signature.)
• Withdraw Form B (For WP/WF) - (Fall 2008 - Due on or before Monday, November 24, 2008 with the Professor's signature and the Department Chairperson's signature.)
• For the Fall 2008 semester, after November 24, the Registrar's Office will no longer distribute this form; it will only be available from the Dean of the College in which the course is offered. During these last four weeks of the semester, withdraw must be considered exceptional and may occur only with the approval of the Professor, Department Chairperson and College Dean and only for good and sufficient reasons beyond the control of the student. WP/WF remains in effect.)
