Additional IACUC Forms In some cases, additional forms will be required prior to working with animals. Please contact the Principal Investigator or Manager prior to completing additonal IACUC forms to discuss what forms are applicable to your work. Employee Animal Worker Health Screening Questionnaire - Click on link and print out. Employee Animal Woker Health Screening Questionnaire should be provided to your health care provider to complete and retain. After the healthcare provider has completed the questionnaire, please ask your health care provider to review and sign Employee Health Screening Certification.
