Sports Physicals & Travel Evaluations
The Rowan University Student Health Services (SHS) requires all incoming freshman and transfer students to submit a complete physical (within 1 year of enrollment) along with the other mandatory pre-enrollment health forms. The following types of physicals are offered to full or part time matriculated students currently enrolled in classes that have submitted their Pre-Enrollment Health Forms and Physical. SHS does not conduct physicals for employment or other non-university related functions. Students that make an appointment for a physical and do not show up or cancel their appointment will be charged a No Show Fee.
Sports Physicals - Please bring your completed Athletic Pre-Participation Health History and Athletic Authorization Form to your appointment. Forms should be completed in pen.
- Pre-Participation Sports Physicals
In partnership with the Athletics Department SHS conducts pre-participation sports physicals on all incoming freshman and transfers each summer. Students will be required to submit all mandatory pre-enrollment health forms before receiving this comprehensive sports physical. If the student is under the age of 18, a legal guardian must sign the required forms listed above. In addition, NCAA requires that all athletes also submit evidence of the following items which must be completed prior to the Pre-Participation Sports Physical:
There is no charge for the Pre-Participation Sports Physical if the student attends the scheduled clinic or makes an appointment for a physical prior to the clinic date. Each coach will communicate the scheduled clinic time with the student athlete. Contact the student’s coach for additional information. These clinics are usually held the last week in July for students entering in the fall and early January for students entering in the spring. Students will be charged for any Pre-Participation Sports Physical scheduled after the clinics.

Returning student athletes will be cleared by the SHS nursing staff unless there has been an injury requiring a targeted physical. Students are responsible to bring documentation of any medical clearances for injuries, surgeries or medical conditions. Clearance physicals clinics will be scheduled in the spring and summer at no cost to the student. If a student requires clearance outside of the scheduled clinics there will be a charge.
Athletic Trainer Physicals - Please bring your completed Athletic Pre-Participation Health History and Athletic Authorization Form to your appointment. Forms should be completed in pen.
Athletic Training students must meet the same requirements as the listed above for the Pre-Participation Sported Physical. This physical must be scheduled.
Travel Evaluations - Prior to your appointment please review our Travel Health Questionnaire and visit the CDC's recommendations for those traveling abroad to the locations you plan on visiting.
SHS provides travel medical physicals and evaluations to eligible Rowan University students. In order to facilitate safe and healthy travel our providers screen for medical conditions which may impact on travel, complete study abroad examinations, and provide necessary vaccinations and prescription medications for prevention of travel related illnesses. Patients will be referred for additional medical services as needed.
Rowan University students are involved in a number of activities which involve international travel. This travel may involve health risks which can be minimized with proper preparation and education. The delivery of appropriate pre-departure care involves specific and detailed information about the location of travel, length of stay, type of setting and anticipated activities. This information must be available to the SHC via the traveler or those arranging and overseeing the travel at the time of a scheduled visit.
Student Health Services charges for travel evaluations. Additional fees may be applicable for referrals, tests, medications or vaccinations that may be ordered based on the students travel agenda.
ROTC physicals are conducted by a provider. These appointments must be scheduled. Students must have their Mandatory Pre-Enrollment Health Forms completed prior to scheduling their appointment. There is no charge for an ROTC physical.
